Admission and registration
The transition to our new electronic patient record will take place on 12 June 2026. From that date onwards, you will find up-to-date information here about registering with the LUMC. Is dit uw eerste afspraak in het LUMC?
Het LUMC heeft nog niet alle informatie van u. U meldt zich dan aan bij de aanmeldzuil bij de centrale patiënten inschrijving in de centrale hal. De inschrijfbalie is open van maandag tot en met vrijdag van 7.30 tot 16.30 uur. Hier controleren we gegevens zoals uw huisarts, tandarts, apotheek en contactgegevens. Ook maken we een foto voor uw medisch dossier. We vragen hiervoor eerst uw toestemming.
Meenemen voor uw inschrijving
- Geldig legitimatiebewijs (paspoort, identiteitskaart, rijbewijs of vreemdelingendocument)
- Verzekeringsbewijs
- Naam en adresgegevens van uw huisarts, tandarts en apotheek
Komt u voor een vervolgbezoek?
U kunt online inchecken (aanmelden) voor uw afspraak. Dat kan vanaf 24 uur voor uw afspraak. Mocht het niet lukken om online in te checken (aan te melden), kunt u in de centrale hal terecht bij een van onze aanmeldzuilen.
Finding your way to the right department
After you’ve reported to the admissions booth, you will easily be able to locate the department where you have your appointment using the associated route number. You will find the route numbers on the information signs in the main concourse or on the website of the department. To see these, go to the overview of the outpatient clinics, nursing wards and specialist centers of the LUMC. Our volunteers in the main concourse are always happy to help you find your way.
Please note: The Poli-building Surgery, Orthopaedics and Plaster Room is in the outer poli-building next to the parking garage. This is building 14 route 1000.

Gebruik ook de routekaart van het LUMC hoofdgebouw: Route van A naar B. Bereid uw bezoek verder voor door meer te lezen over de route naar het LUMC en mogelijkheid tot parkeren.
Kom ruim op tijd
Zorg ervoor dat u minimaal 15 minuten voor uw afspraak bij het LUMC bent. Komt u met de auto? Kom dan minimaal 30 minuten van tevoren, het kan druk zijn bij onze parkeergarage. Zo heeft u voldoende tijd voor het parkeren, aanmelden en/of inschrijving en het lopen naar de afdeling van uw afspraak.
Als u voor een vervolgafspraak komt of al eens in ons ziekenhuis bent geweest, kunt u alvast online inchecken via mijnLUMC. Dan kunt u bij aankomst in het LUMC direct door naar de afdeling waar u uw afspraak heeft.
Due to hygiene and risk of infection, pets are not allowed in LUMC. There is an exception for assistance dogs.
Your assistance dog is welcome in the LUMC. However, your dog must be a certified assistance dog. In addition, the assistance dog must be recognisable by a cover with an officially recognised logo of the Stichting Hulphond or KNGF Guide Dogs.
Are you a patient and would you like to bring the assistance dog to your appointment? Please contact your attending physician before your appointment. This is because the department may have a patient with reduced resistance.
