Your visit

Have you come to the Leiden University Medical Center (LUMC) for an appointment or treatment? If so, please report to one of the admission booths in the hospital’s central area. Is this your first appointment at the LUMC?

If this is your first appointment at the LUMC, then please go to the registration desk to have yourself registered. This way, we’ll be able to help you as best we can. Has your situation changed? You can always amend your personal information. Use the route numbers on the information signs to find your way to the right department.

Come on time

Make sure you arrive at your appointment at the LUMC at least 15 minutes early. Taking the car? Then make sure you arrive at least 30 minutes early. This way, you will have enough time to park your car as well as deal with admission and registration.

Admission and your first appointment

Before your appointment, you need to report to one of the admission booths in the main concourse. There, you will be given a ticket. You can use this ticket for the admission booth in the department where you have your appointment.

Please note: The poli-building for Heelkunde, Orthopedie en Gipskamer is in the outer poli-building next to the parking garage. This is building 14 route 1000. In that case, you don't have to report to the admissions booth in the main building. Instead, you can report to one of the admissions booths in the poli-building.

Is this your first appointment at the LUMC? First, please report to one of the admission booths in the main concourse. There, you will be given a ticket that will be used to summon you to the registration desk in the main concourse, where you will be registered.

The registration desk is open from Monday to Friday from 7:30 to 16:30. At registration, your photograph will be taken for your medical file. This way, our care providers will know which face belongs to which file. But first, we ask for your permission in this regard.

To be taken along for registration

  • Valid identity document (passport, identity card, driver's license or immigration document)
  • Insurance certificate
  • Name and address information of your family doctor, dentist and pharmacy

Finding your way to the right department

After you’ve reported to the admissions booth, you will easily be able to locate the department where you have your appointment using the associated route number. You will find the route numbers on the information signs in the main concourse or on the website of the department. To see these, go to the overview of the outpatient clinics, nursing wards and specialist centers of the LUMC. Our volunteers in the main concourse are always happy to help you find your way. 

Please note: The Poli-building SurgeryOrthopaedics and Plaster Room is in the outer poli-building next to the parking garage. This is building 14 route 1000.

Transmission of identity document and citizen service number (BSN)

The LUMC is legally obliged to establish the identity of all patients. Even newborn babies and children require an identity document. You should therefore apply for these documents as soon as possible following the birth. Always take along the identity document of the person with the appointment at the LUMC. Haven't got a driver’s license yet? Then take along your citizen service number (BSN).

The LUMC will register your citizen service number (BSN). We are required to do so. We use the citizen service number to exchange information concerning you with other healthcare providers. For example, your family doctor and dentist. We also pass your citizen service number on to healthcare providers. If we do not do this, then the healthcare provider might not pay your medical bill. If this is the case, then you are required to pay the costs yourself.

Due to hygiene and risk of infection, pets are not allowed in LUMC. There is an exception for assistance dogs.

Your assistance dog is welcome in the LUMC. However, your dog must be a certified assistance dog. In addition, the assistance dog must be recognisable by a cover with an officially recognised logo of the Stichting Hulphond or KNGF Guide Dogs.

Are you a patient and would you like to bring the assistance dog to your appointment? Please contact your attending physician before your appointment. This is because the department may have a patient with reduced resistance.

Amending information

Are you moving or has there been a change in your address or personal information? If so, then notify the Central Registration Department via email or by telephone. Always provide your patient number, name and date of birth.

Amend information via mijnLUMC

You will receive a confirmation email from our department once the changes have been processed. Do you only want to amend your email address, telephone number or pharmacy? You can easily do this yourself in the mijnLUMC patient portal.

Add your child to your healthcare policy

Newborn children are added to their parent’s healthcare policies. You can provide your policy number to the department secretary during admission and registration at the LUMC.
