AGB-code
The General Register of Healthcare Providers (AGB) records and codes the details of healthcare providers in the Netherlands. Do you work in the healthcare sector? Then you need an AGB code for identification in the various (digital) processes within healthcare.
An AGB code is a unique code that you need for identification in the various (digital) processes within healthcare. The code shows your personal details and authorisations. It allows institutions to look up your details, for example, for referrals or prescribed medication.
We need your AGB code to claim the cost of your care from your insurer. You can read more about this in the General Data Management Register on the AGBcode website.
Who is required to have an AGB code?
An AGB code is mandatory for all healthcare providers, including medical specialists, physiotherapists, paramedics, etc. A complete overview of all professional groups can be found on the AGBcode website.
The AGB code is not intended for:
- AIOS
- ANIOS
- Physician Assistants
- Co-assistants
- Nurses
- Nursing staff

Occupational disability insurance (AOV)Occupational disability insurance (AOV)
If you become (partially) disabled, your income may decrease. This is also the case if you receive WIA benefits. To limit this risk, you can take out a supplementary insurance with Loyalis: the occupational disability insurance (AOV). This insurance replaces at least 70% of your income until you reach state retirement age. Our employees receive a discount on this insurance.
If you are ill for more than 2 years, your employment will be terminated. However, you may be entitled to WIA benefits and an ABP disability pension. Despite these benefits, your income will almost always fall. The AOV allows you to take out additional insurance against this risk. It also allows you to continue to build up a pension on the part of your income that you have insured.
How does the AOV work?
You can choose between three different types of cover:
- Loss of income in the event of partial disability;
- Loss of income in the event of total disability;
- Loss of income in the event of both partial and total disability.
On the Loyalis website you can read what these coverages include and what conditions apply. There you will also find a test to see what the financial consequences of disability could be for you.
Do not wait too long to apply, as the "no health questions asked" promotion is only available for a limited period.
The cost
You pay the AOV premium yourself. This premium is lower than with other insurers because we have agreements with Loyalis. The exact amount of the premium depends on the cover you choose. You can calculate this premium on the Loyalis website.
Cao umc
As an employee of LUMC, you are covered by the Cao Universitair Medische Centra (Cao UMC). This contains clear agreements on salary, allowances, compensation, holidays and leave. At LUMC you will benefit from very favourable employment conditions. In addition to your monthly salary, you will receive a holiday allowance of 8% and a year-end bonus of 8.3%. You can also count on an excellent commuting allowance and a solid pension scheme with ABP, where LUMC pays no less than 70% of the pension premium. This means that your net salary will often be higher than you initially expect.
All positions have a minimum and maximum salary range. Based on your education and experience, we will agree your salary together.
*From 1 July 2025, salaries will increase by 3% to the maximum of scale 11.

Internal rules at LUMC
In addition to the Cao UMC, we also have a number of internal rules. These contain additional agreements specific to LUMC in the area of personnel and organisation. These rules apply to all employees with a contract of employment at LUMC.
UMC Health Insurance
You and your family members can benefit from the UMC health insurance scheme. This is a collective health insurance policy taken out by the University Medical Centres on behalf of all employees. UMC Health Insurance offers you and your family members a group discount on the basic insurance and supplementary packages. As a staff member, you will receive an additional premium discount on the supplementary packages.
Who can take out UMC Health Insurance?
You can take out UMC Health Insurance if you:
- Have a contract of employment with LUMC, regardless of your level of employment;
- Are a junior doctor in training to become a consultant (aios).You can continue to take out UMC Health Insurance throughout your training at LUMC and continue to receive discounts. You can also do this if you are seconded or temporarily employed in a general hospital during the peripheral period of your training.
You can also take out UMC Health Insurance for family members, such as:
- Your husband, wife or registered partner;
- Your partner (if you live together on a permanent basis and have a notarised cohabitation agreement);
- Children up to the age of 30 who are part of your household.
What are the premiums for UMC health insurance?
You pay a nominal premium for the UMC health insurance. You will receive a discount on this premium. You can find an overview of the premiums and discounts on the UMC Zorgverzekeringen website.

Collective discount system for non-life insurance
LUMC has entered into a collective discount agreement with Centraal Beheer Achmea for non-life insurance for its employees. This offer from Centraal Beheer Achmea applies to all employees who have an employment contract with LUMC. It also applies to volunteers who have a contract with us and to trainees who are in training with us. Of course, you are not obliged to take advantage of this offer.
We chose Achmea because of:
- The competitive price-quality ratio;
- the fast processing of claims;
- the ease of taking out and cancelling insurance;
- the adequate provision of information.

- the amount of the discount depends on the type of insurance.

Optional pension
As an employee of LUMC, you are entitled to a pension from Pensioenfonds ABP. This is called the ABP voluntary pension. You can start this pension at any time between your 60th birthday and five years after you reach the AOW pension age. The longer you work, the higher your pension will be. The elective pension is paid on top of your AOW pension and any pensions you may have earned from other employers.
In principle, our employees retire on the day they reach AOW pension age. If you want to retire early or work fewer hours, you should discuss this with your manager. The early retirement pension allows you to choose when you retire. At that time, you can choose to retire either fully or partially. If you take partial retirement, you will only build up a pension for the hours you continue to work.
How much pension will I get?
The amount of your pension depends on how much you have earned and how much pension you have built up during the years you have worked. It also depends on when you stop working. The longer you work, the higher your pension will be. You can also vary the amount of your pension.
How can you increase your pension?
You can increase your elective pension by adding your survivor's pension. Every employee builds up this survivor's pension with ABP. If you wish, you can exchange this pension for a higher elective pension just before you reach state retirement age. This concerns the survivor's pension that you acquired after 1 July 1999. If you are single, the ABP will make this exchange for you automatically. If you have a partner, you decide together whether you want to make use of this option.
The costs
The premium for your optional pension will be deducted from your salary. You will find your pension premium on your payslip.

Transport and mobility facilities
Commuting Expense Scheme
Travel with your own transport
Do you travel to work in your own car? Once you are employed by us, you will be reimbursed €0.18 per kilometre up to a maximum of 40 kilometres per trip. Do you cycle or walk to work? Then the mileage limit does not apply and you will receive €0.18 per kilometre for the full distance. In addition, you can take advantage of an extra tax-exchange scheme for €0.05 per kilometre. This makes sustainable travel even more attractive!
100% reimbursement for public transport (commuting distance of 7 km or more)
Do you live 7 km or more from work? If so, you will be reimbursed 100% of your public transport costs without having to pay anything. As an employee, you will also receive an NS Business Card (2nd class) to help you get to work. Once you have been recruited and your employment is complete, you will receive an email with instructions on how to apply for your Businesscard. Please note that you will not be able to apply for a tax swap on the days you use the Businesscard, as you will already have been fully reimbursed.
Travel costs by public transport (commuting distance less than 7 km)
Even if you live closer, we will make sure your travel costs are covered. If you live within 7 km of work, you may not qualify for the full public transport allowance, but you will still be reimbursed €0.18 per kilometre. You can also take advantage of our tax exchange scheme, which gives you an extra tax-free voucher.
Tax Exchange
In addition to our standard travel allowance, once you are employed we offer you the opportunity to get more out of your salary through our tax swap scheme. This means you can exchange part of your gross holiday allowance or year-end bonus for a tax-free commuter allowance. This means you keep more net. The scheme is completely voluntary and open to all employees. Do you use an NS Business Card? Then you already have the maximum tax-free reimbursement and this exchange scheme does not apply.
Combinations are possible
We believe that flexibility is important. It is therefore possible to use public transport one day, drive or cycle to work the next, and even work from home one day (if it suits you). In this way, everyone can choose the option that best suits their situation and activities on a day-to-day basis. It is not possible to request different transport options and/or work from home on the same day.
Allowance for working from home
Do you work from home one or more days a week? We pay you €2.40 for each day you work from home, to cover your costs for things like internet and coffee. Once you are employed, you can easily register your home working days in our travel calendar to receive this reimbursement. This makes flexible working from home even more attractive.
Bycicle and parking facilities for staff members
For our employees, we have a special bike scheme that gives you a tax benefit when you buy a new bike or e-bike. Even your bike insurance and other essentials are covered. And if you cycle to work, you can park your bike in one of the (guarded) covered bike sheds.
Cycle parking
At LUMC you can park your bicycle or moped in a number of different areas. Under each of the three LUMC buildings there is a staff bike park.
Students can only use the bike park under Building 3 (Education Building). There is also parking for motorcycles, scooters and cargo bikes. The staff and student car parks are secure and only accessible with a staff badge. Parking is not allowed outside the racks.
Cycle parking for staff
- Cycle shed building 1 (staff)
- The bike shed is located in the basement on the forecourt of the LUMC.
- Cycle shed building 2 (staff). The bike shed is located in the basement of Building 2, the Research Building. There are 442 spaces available.
- Building 3 (staff and students). Located in the basement of Building 3, the Education Building. There are over 1,500 bicycle spaces available.
On your first day
On your first day of work only, you can park your bicycle or moped in the LUMC visitor car park. If you have your staff badge, you can park your bike in one of the secure spaces.
Parking for employees
As an employee you can use the car park for a fee. LUMC has three different rates for this. Which rate you pay depends on where you live, the day and the time. The different rates are detailed below.
From 1 November 2023 the following rates will apply:
1. Weekend Evening Night Rate (WAN) | all employees will be €1.65 per day from 1 November 2023 The WAN rate applies to all employees, regardless of the postcode you live in, at the following times:
- Weekdays between 13:30 - 05:00
- Saturday & Sunday Bank
- Holidays (including 3 October)
2. Staff rate | staff within the postcode restriction
If you live relatively close to LUMC, it is possible that your postcode area falls within the LUMC parking restriction. This means that at certain times of the day you will have to pay the visitor rate to use the LUMC car park. This is a measure to ensure that there are enough parking spaces for our visitors and patients.
- Mon-Fri: enter between 5.00 and 13.30 for the visitor rate.
- Mon-Fri: enter between 13.30 and 5.00 for the WAN rate.
- Sat-Sun: the WAN rate applies all day.
- Bank Holidays (incl. 3 October): the WAN rate
If you work an evening or night shift, you can park at the WAN night rate until 09.30 the next day.
3. Employee rate | employees outside the postcode restriction
The employee rate for employees outside the postcode restriction applies weekdays between 05:00 - 13:30. Weekdays between 13:30 - 05:00, Saturdays, Sundays and public holidays the WAN rate applies.
- Sat-Sun: all day the WAN rate applies.
- Public holidays (incl. 3 October): the WAN rate
If you have an evening or night shift, you can park at the WAN rate until 09:30 the next day.
Aanvullende voorwaarden bij indiensttreding
Functies en opleidingen in de directe patiëntenzorg
Ga je werken of een opleiding volgen in de directe patiëntenzorg? Dan moeten we vaststellen of er in jouw functie gezondheidsrisico’s voor je zijn. Daarvoor hebben we de volgende documenten nodig:
- een ingevulde en ondertekende Verklaring eigen gezondheid
- een kopie van je vaccinatieboekje met gegevens over hepatitis B-vaccinatie (datum en titeruitslag)
- een kopie van je vaccinatiegegevens, op te vragen via het RIVM
Mail deze documenten naar vgmgezond@lumc.nl. Van de afdeling Gezondheid ontvang je meer informatie over het vervolg van het intrede onderzoek.
Medische Keuring
De Raad van Bestuur LUMC en het College van Bestuur van de Universiteit hebben in principe de aanstellingskeuring afgeschaft. Zij hebben besloten dat keuringen (door de afdeling VGM) alleen nog kunnen worden uitgevoerd als het nut en de noodzaak van de keuring kunnen worden aangetoond. Dit is bijvoorbeeld het geval bij lichamelijk belastende functies en functies waarbij een veiligheidsrisico bestaat.
Van de volgende functies is aangetoond dat een keuring nuttig en noodzakelijk is:
- Leerlingen: verpleegkundige, operatieassistent, anaesthesiemedewerker, radiodiagnostisch laborant
- Radiologisch werkers categorie A
- Persluchtmaskerdragers bij de bedrijfshulpverlening
- Vacciniawerkers: medewerkers die onderzoek doen met vacciniavirus
- Snijdend specialisten: o.m. chirurg, orthopaedisch chirurg, KNO-arts, gynaecoloog, dermatoloog
- Anaesthesist, perfusionist, operatieassistent, anaesthesiemedewerker
- Verloskundige
- Medewerkers Centrale Sterilisatie Dienst.
MRSA-screening: alvast zelf doen?
Heb je bij het invullen van de Verklaring Eigen Gezondheid één van de vijf vragen over MRSA met ja beantwoord? Neem dan zo spoedig mogelijk contact op met de afdeling Gezondheid (071-5268015) voor het afnemen van MRSA screeningskweken. Doe dit uiterlijk 4 weken vòòr je eerste werkdag in het LUMC.
Je mag niet werken in de directe patiëntenzorg tot je deze test hebt ondergaan en deze een negatief resultaat heeft opgeleverd. Je kunt ervoor kiezen voor je vertrek naar Nederland zelf alvast een MRSA-screening te laten uitvoeren. Als deze negatief uitvalt en niet ouder is dan 14 dagen, mag je alvast in het LUMC aan het werk. Op de afdeling Gezondheid zullen echter opnieuw MRSA-screeningskweken worden afgenomen.
MRI-screening
Ga je werken in MR-ruimtes? Vul dan het screeningsformulier in en stuur dit op naar de afdeling Veiligheid, Gezondheid en Milieu via e-mailadres secretariaat_vgm@lumc.nl. De bedrijfsarts beoordeelt aan de hand van de screening of je veilig in de MR-ruimten kunt werken. Bij een contra-indicatie voor het werken in MR-ruimtes, neemt de bedrijfsarts contact met je op.
Arts-assistenten
Voor de arts-assistenten (AIOS en ANIOS) geldt in het LUMC de Instructie Arts-assistenten en de regeling Supervisie Arts-assistenten. Word je als arts-assistent aangesteld? Dan word je geacht deze regelingen te kennen.
Onderzoekers in Opleiding / promovendi
Krijg je een arbeidsovereenkomst als onderzoeker in opleiding (OIO), dan is het OIO reglement van toepassing. In dit reglement zijn specifieke afspraken over je contract, opleiding en begeleiding, beoordeling, arbeidsvoorwaarden en het einde van de opleiding vastgelegd. Lees dit reglement goed door.

Wetenschappelijk onderzoekers
Ga je wetenschappelijk onderzoek voor het LUMC verrichten, al dan niet aangesteld als onderzoeker? Dan geldt voor jou de informatie over Wetenschappelijke Integriteit in het LUMC.
Medewerkers met een nul-urenarbeidsovereenkomst (oproepkracht)
Krijg je een arbeidsovereenkomst als medewerker met een nul-urenarbeidsovereenkomst (oproepkracht)? Dan zijn de reguliere arbeidsvoorwaarden voor jou van toepassing. Daarnaast gelden er bijzondere arbeidsvoorwaarden die voor oproepkrachten van toepassing zijn. Deze bijzondere arbeidsvoorwaarden staan beschreven in de Interne Regeling 004 van het LUMC.
Het aantal uur dat je werkt, is niet vastgelegd en kan van week tot week verschillen. Met je leidinggevende of je poolmanager spreek je af op welke dagen, perioden en tijden je inzetbaar bent. Als je leidinggevende je op deze uren oproept, dan ben je verplicht om te komen werken. Je wordt minimaal 24 uur van te voren opgeroepen. Anderzijds kan het ook voor komen dat je niet opgeroepen wordt.
Vakantie en vakantiegeld
Bij een nul-urenarbeidsovereenkomst krijg je je vakantie-uren een keer per jaar uitbetaald. Over de uren die je werkt, bouw je bovendien 8% vakantiegeld op. Dit wordt elk jaar uitbetaald bij je salaris in mei.
