Discharge from a nursing ward

Your discharge from the LUMC

Are you permitted to leave the nursing ward to go home or to a care facility? We refer to this as your “discharge” from the LUMC. There are a number of important matters to consider upon your discharge. These include aftercare to facilitate a faster recovery and medication for after your admission.

Your discharge date

Prior to your admission, you will be informed of the expected date of your discharge from the hospital. If this is not the case, a doctor or nurse will discuss this with you during your admission. This usually takes place 1 or 2 days before your discharge. We will also discuss with you in advance whether your discharge will require further care.

The day of your discharge

Discharge consultation

Before you leave the LUMC after your admission, you will have a discharge consultation. During this consultation, we will discuss, for example, what you need to pay attention to after your admission and what you can and cannot do (for the time being). You will also have the opportunity to ask questions. Please let us know if you would like a loved one to be present. We will take this into account.

Advice and medication

Upon discharge, you will receive medical advice and more information about your medication. Always ask for a medication overview. This will ensure that you know which medications you need and how to use them.

Follow-up appointment

After your discharge, you will have a follow-up appointment. You will receive this appointment by mail and digitally via MijnLUMC. If you do not have transportation home, ask the nursing staff to call a taxi for you. After your departure, we will also inform your primary care physician (or other healthcare providers) about your treatment and the necessary aftercare.

At LUMC, we are constantly working to improve our care. You and your experience play an important role in this. Would you like to share your experience of LUMC with us? If so, please complete the BeterMeter. This is voluntary and anonymous. We use the BeterMeter to collect patient experiences and improve care at LUMC.

Points to consider after your admission

Follow the instructions you receive during your discharge consultation. If you experience any symptoms related to the procedure, please contact the department where you were treated or the nursing ward. You will be given a telephone number for this purpose when you leave the LUMC. If you hear a voicemail message with instructions when you call, please listen to it in its entirety.

Care after your admission: Transfer Office

You may still need help and/or care after your discharge. The LUMC Transfer Office can help you with this. During your admission, your doctor and a transfer nurse will discuss with you any care you may need after your treatment or admission. Your wishes and options will be discussed. You can always involve a family member or caregiver in this process.

There are things you can arrange yourself and things that the Transfer Office will request for you. The transfer nurse will find care and/or a location that suits your needs. We will do our utmost to meet your wishes. This depends, for example, on the available capacity of the aftercare location.

In order to arrange care, the Transfer Office must share your medical data with care providers outside the hospital. This is only permitted with your consent. That is why we ask for your verbal consent after consultation with the lawyer.

Making arrangements after your admission

Do you anticipate needing assistance after your admission? If so, discuss in advance with family and friends who can assist you. For example, with cooking, shopping, and housework.
