Your appointment interview
In order to complete your appointment at the LUMC, we require certain data. A number of forms and documents is needed for all appointments. These are listed below. Of empoyees in certain professions we require additional information. Please refer to the page Additional appointment conditions to see if these apply to you. Please read both pages carefully.
Bring or send your documentation?
You may bring the required documents and forms with you to your appointment interview, unless stated otherwise. Is an appointment interview not required? Or are you staying abroad prior to your appointment at the LUMC? Please send the required documents to your P&O department (by e-mail) and bring the original documents with you on your first working day/ to your appointment interview after arrival in the Netherlands.
Note: please provide clear, legible copies or scans of the requested documents. Photos of documents taken with a cell phone, are often difficult to read and shall not processed by us.

General personal data
In order to complete your appointment at the LUMC, we require certain personal data and details about your work history. Please bring the following:
- Questionnaire for new employees (completed and signed, please note: 5 pages)*
- Your valid passport or ID card (driving license will not be accepted).
*To save the filled-in form, click 'Print' and change the destination to 'save as pdf'.
Certificate of Good Behaviour (VOG)
In order to work at the LUMC, you must provide a Certificate of Good Behaviour (VOG). Where and how to apply for a VOG depends on your country of origin and the duration of your stay in the Netherlands. For more information, please refer to the personnel department that arranges your appointment. The cost of the VOG will be reimbursed on presentation at the first salary payment or, if you do not receive salary, through the financial administration.
Please note: if you cannot provide a VOG, the LUMC will not be able to appoint you.
Diplomas
At the appointment interview, you will be asked to show your original diplomas and/or certificates (or certified copies thereof), which are relevant to your profession.
PhD students with a college or foreign degree must fill in an Admission form. With the form, please provide the following documents:
- Certified copies of your bachelor's and master's degree,
- Certified copies of your transcripts,
- A statement by the supervisor that accepts you as a candidate.
Are your certificates drawn up in a language other than Dutch, English, German or French? Please provide certified copies of the English translations as well. To read more about doing a PhD course at the LUMC, refer to the page Graduate School.
Registrations
Does your position require a professional registration? Please bring your registation certificate with you to the appointment interview.- BIG registration: register of healthcare professionals*
- RGS registration: register of medical specialists
- RSV registration: register of nurse specialists
- Training Registration: register of medical assistants in training
- BKO registration: university teaching qualification
- SKJ-registration: register of youth professionals
Summary of ancillary activities
Besides your appointment at the LUMC you may, under certain circumstances, employ additional activities. If these are in line with your work at the LUMC, then you need permission. Download the form Ancillary activities (pdf) or refer to the page Ancillary activities for more information.
Commuting expenses
Paid employees of the LUMC can partake in a tax scheme for commuting expenses. For more information, please refer to the page Commuting expenses scheme . Should you want to participate in this scheme, please fill out the application form and bring it with you to your appointment interview.
